There is an additional $5 dollar fee for completion of forms for use outside of the patient’s medical record, including:

  • Day Care

  • Head Start Physical Form

  • Sport Physical Form (Participation physical evaluation Form)

  • Pre-Employment

  • Life Insurance.

  • Family Medical Leave
