We provide comprehensive health care from birth to 18 years of age, including health checks, school physicals and immunizations. We accept Medicaid and welcome new patients with a commitment to provide exceptional care for your child’s healthcare needs.

Patient’s Intake Form
Patient’s Intake Form is needed If you are Established patient and being seen for health check, or a new patient (regardless of the reason for the visit). Please fill out online and submit before your office visit.

Appointments
If you are unable to keep your appointment, please notify us at least one day (24 hours) prior to your appointment. Doing so, will enable other patients to have an appointment. We and other patients will appreciate your courtesy.

We accept cash, checks, MasterCard, American Express, Visa and Discover cards. You may pay your bill using PayPal without having PayPal account.
Medication Refill
You may request refill on your child regular medications, some medications cant be refilled unless you speak to our staff directly.

Confirm (or Cancel) an appointment
If you are unable to keep your appointment for any reason, Please notify us at least one day (24 hours) prior to appointment. this will allow us to accommodate patients in timely manner and help other patients have an appointment. Any unconfirmed appointment will be rescheduled. We will no longer give appointment to patient with repeated no shows.

Due to COVID-19, and to protect our staff members, their families and susceptible patients, Face Mask is required for all caregivers and patients older than 2 years with fever or history of exposure to COVID-19.
The child should be accompanied by only one adult (any additional family members may stay in personal car, if possible, while patient is being seen).
All our forms are available on line on our website and can be filled out and submitted remotely using personal computer, iPad or smart phone.

All scheduled appointments must be confirmed ONE DAY before appointment’s date by latest 12 noon. (all patients must have confirmed appointments so we can triage and accommodate patients safely).
Arriving on time allows us to provide you with a service in timely manner which decrease waiting time before patient is seen.
We expect patient and parent to stay in examination room with door closed during the entire office visit.
Health check visits can be scheduled only for the morning (7:30 am – 12 noon). Sick patients can be seen only in the afternoon (1 pm – 4:30 pm). This will help prevent the spread of infections.
We respectfully request that you follow these guidelines to ensure the safety of patients and staff during COVID-19 epidemic. We apologize for any inconvenience this may cause.
